ATTENTION: Undergraduate International Students

Medical Consent Form Required for Students Younger than 18

It is quite normal for newly arrived students to become ill after a long trip, dealing with time changes, new food choices and a fair amount of stress. MSU Student Health Services is on campus to provide medical care if or when the need arises. In the State of Michigan, students must have the consent of their parent or guardian to receive non-emergency medical care if they have not yet reached the age of 18 years. If you were born after August 16, 1994 your parent or guardian must fill out and return a Medical Treatment of Minor Students form (PDF)* (http://olin.msu.edu/pdfs/CONSENTFORMINORSTUDENTS.pdf) in order for you to receive medical services. (Emergency medical services will be provided without consent.)

The form should be printed, completed and signed by your parent or guardian, and returned to MSU Student Health Services. You can bring the signed form with you to MSU and deliver it to the Olin Health Center Medical Records department room 146. Other options are to have your parent or guardian mail or FAX the form to Olin Health Center. Questions may be emailed to olin@msu.edu

It focuses on the non-academic issues that have the most impact on the academic success of MSU students.
